What is the need?
People who need palliative care are too often lost in a fragmented system of silos of practice, inconsistent referral pathways, and unclear eligibility criteria.
What is PCV asking for?
Dedicated funding for PCV to lead statewide integration of referral and transition pathways, working across sectors to align eligibility criteria and streamline access between hospitals, primary care, aged care, and community services.
Statewide communication and marketing campaign to raise awareness of the new pathways among clinicians, referrers, and the community, ensuring people know when and how to access palliative care.
